if you're finger's getting swollen and hurts like hell than you should go to a doctor and he'll poke a hole in the nail to relieve the pressure of the blood. normally the nail doesn't fall off if you do that. I did it when it got to the point where I couldn't even bend the finger so I don't know what point makes it needed.

you can do it on your own as well. Take a paper clip, un-bend it. And heat it up and stick it on a nail and burn a hole in it.
