No, you do not need to use the tool. Licenses are transferred automatically during the repair process but you may need to download them again. Follow the instructions at the Console After Repair page.

Why can I use the tool only once per twelve months?

The tool is intended to allow consumers to consolidate their licenses they purchased on multiple consoles to a primary console. It is not intended to continually move content between multiple consoles.

All it does is tell you what the reps tell you, redownload your content.
except that you won't have to be signed in live to play them after using this, right?

ok, i tried it and all my xbla are working offline and online.

you need to redownload them though, go to your download history or the arcade store, just redownload, no need to delete.

Once every twelve months?  Give me a fucking break.  People have gone through several system replacements in less time than that.  What a jackass rule that is.

12 months is a stupid (and seemingly pointless) rule, but I'm looking forward to being able to play some of this crap offline such as Shivering Isles.
