What does "by hand" imply? I've been in web design since it was forced on me a decade ago, and I refuse to venture anywhere near anything that isn't wysiwyg.

there's nothing wrong with using an editing program that makes all the functional garbage less work

Notepad is all a man needs.

But I use Visual Studio. Intellisense, you know. And the fact that is closes tags for you as you create them. So convenient.

I use Wordpad. Nothing like multiple-level undo!

Yeah it's pretty hard, but not all that hard once you get used to it.  Then it's just wrestling with browser problems  :maf

Textmate is great on the mac... Coda seemed interesting also.  I used Notepad++ a bit on windows and it seemed fine.
I don't use any of that WYSIWYG stuff  :-X

My site used to be done in Notepad. Then I starting using Wordpress and I've got no idea why I would use anything else.
