I don't think I've seen a manga treat it's main character to this much pain anguish and misery as I have in Shamo. It's like Hajime No ippo for twisted fucks.

[edit] Part 1: The reformatory
Ryo, a talented highschooler who was about to enter Tokyo University, the most prestigious university in Japan, killed his parents before his successful life could begin. The perfect family and perfect life seemed to devour the young boy's restless soul. In a beautiful sunny afternoon when cicadas were singing, he stabbed his father and mother to death with a short knife repeatedly before he came to his senses.
The 16-year-old bookworm murderer was convicted and sent to a reformatory where he was gang raped by other boys. Kenji Kurokawa (黒川健児 ?), a jailed man who nearly assassinated Japanese Prime Minister decades ago, was sent to the reformatory to teach the youngsters karate every week. He discovered Ryo's talent and taught him self-defense. Ryo survived. Thanks to a law that protects minors, he was released two years later.
Alone in a city full of crime, he tried to look for his lost beautiful sister who, because of the tragedy, became a homeless drug-abusing prostitute. Ryo started to fight for his living and used all imaginable dirty tricks to defeat his enemy. He also ambushed gangsters in dark alleys to perfect his fighting skill as well as working as a gigolo.

there's a recent movie adaptation out (http://www.amazon.com/Shamo-Ryo-Ishibashi/dp/B001HRJFXI/ref=pd_bbs_sr_1?ie=UTF8&s=dvd&qid=1238018978&sr=8-1). Its been on my Netflix queue for a awhile, but its status never changes from "Short Wait".
The director's previous film was the nearly unbelievably brutal and bleak (and also highly fucking awesome) Dog Bite Dog, so I've got high hopes for it.

true dat. Casting him as an emotionally stunted, vacant killer was pretty brilliant.
I loved that movie, I only wish the ending wasn't so off the rails crazy, but until then its easily among the best Hong Kong films in years.

gantz kind of spreads the pain around and it's mostly physical
here, the physical aspect is pretty rough but the psychological trauma is worse
there are times where you can see the gears operating and you realize "oh, it's basically a shonen manga for headcases" but if you think about the guys sitaution at the start and then to the later bits, it's a really fucked up "hero's" journey

I love Berserk.

Dude I just got up to date on this manga like, a week ago. Dude.....I....It's just. :-\ Just you wait, it only gets better/worse. It's the most F'd up manga I've ever read I'd say. :'(

If you want one thats in a similar IMO you can check out Koroshiya Ichi (Ichi the Killer) if you havent already
It has a lot of fucked up stuff in there too:
a main character that acts like an idiot but can kill you (in the most grusome ways) like "that"
another character that is a sadomasochist yakuza enforcer with a Glasgow smile
dude raping a dead body
substance abuse
etc
There is a Japanese movie for it but read the comic first

saw the movie last night, and I liked it. Didn't love it. Didn't quite add up to the sum of its parts. There's some really good supporting performances (its got the always terrific Francis Ng, so that's natural), plenty of well staged and reasonably brutal fight scenes, doesn't shy away from its fucked up material, and is a well-made film in every technical sense. But it rushes through so much plot in its 105 minutes that everything feels rushed and the characters feel flat. And the lead (Shawn Yue) gamely does his best but comes off as a cipher. He's been better before, but in this movie he only works part of the time.
Still not a waste of time by any means, just not the slam dunk it could have been.

I stumbled across an interesting little manga that fits the bill. It's called Franken Fran and it's about Fran, a freakish monster/medical doctor/brilliant scientist who attempts to help people solve problems in their lives by employing her unique brand of medicine. It's mostly episodic and every chapter generally ends with everyone learning a valuable lesson and/or falling victim to some gruesome fate. It's oddly light-hearted at times [mainly because of Fran's positive outlook, regardless of how horrifying the situation is], in stark contrast to the explicit, gruesome, sadistic violence that pervades the series.
