I see a person just drive straight through a red light at full speed like once a week. People here are terrible drivers.
Also I'd guess USA has a worse issue with drinking & driving causing bad drivers to be even worse when under alcohol than other countries. Most countries I've been to have basically 0 tolerance for drinking while driving but the USA is like "hey, you can have a drink or two and then drive home"
Also a lot of other countries have public transportation which helps.

Probably due to more rural area/drivers and more areas that have harsh winters? I remember reading a long time ago that you are twice as likely to die in a vehicle crash in a rural area vs an urban area.

The process to acquire a license is also way more strict in Germany for example.

Obtaining a drivers license is much harder and more comprehensive than what is required in the U.S. Also, once you get a drivers license, the Department of Motor Vehicles (Which is run state by state BTW) will pretty allow you to skip tests, either the written test or driving tests for license renewal. I got my license when I was 18 and only had to do a writing test once, and a vision test once. After other time I renewed my license, I either renewed by mail or online.

There is a level of contempt american drivers have for just about everyone and everything else on the road that's really unmatched. Some weird selfish psychology at play where they feel like they're the only one that deserves to be there. I can't find it but I remember seeing some article too that pointed to cyclists being universally hated for ??? reasons as well. Like even the most average or nicest driver being absolute shitbirds to a cyclist if they feel they can get away with it and it won't inconvenience them.
Anecdotally pretty much everyone i know who is a serious cyclist has stories of being fucked over or injured by a driver purposely .

australia drivers are fairly fucking shitty and i have never felt more unsafe riding my bike rurally than there. for some god damn reason, drivers in australia will pass me within inches even when the opposing lane is free of traffic. we cyclists do not fucking understand it but it probably stems from a high level of aggression from australia generally being a shitty place to drive in. the country has shit infrastructure in all the cities i've been to save for certain parts of melbourne. fucking sydney is the worst

there was a recent thesis paper that correlated gun ownership with vehicle size and how those who tend to own guns are more likely to own vehicles on the larger end of the scale and how they tend to treat those said vehicles as not only as an extension of themselves but as a weapon in how they choose to operate it.
it reminded me of another paper i read some time ago about the psychology of humans and how a good number of us, like a good quarter or so, will steer towards a squirrel or any animal when on the road rather than away and the reason being is that some of us are still in tune with our primal nature and those who exhibit this behavior tend to see their vehicle as an extension of themselves so they are naturally inclined to want to "kill" anything smaller.

Brisbane's cycling infrastructure is improving rapidly. At least on the side of the city that I live. However, people are still fucking stupid and think that driving really close to someone in their 2-tonne death machine is funny or cool.

a couple times ago when i had to renew my license in person there was a kid there taking the test, he took it three times while i waited and kept failing it to where the woman basically was telling him the actual answers and he'd forget them when he took it again somehow

FWIW, the US still has worse rate of deaths per vmt than most high income countries, although significantly less bad than with the per capita ratio and in line with some countries like belgium.

That report ToxicAdam posted is interresting, seems to basically boil down to the US has a high ratio of traffic on (non-highway ?) rural roads, which seem to be the most dangerous road type in pretty much all countries.

The thing about rural roads is that you have no protection if something goes wrong. All it takes is one person to go left of center and then you're fucked. You either hit head-on with the other person or you veer off and catapult your car into the air because you hit a drainage ditch next to the road (going at 40+ mph). Striking a tree or flipping your car and killing yourself.
I moved out to central Ohio 15 years ago from the burbs and every year I hear of 2-3 accidents that happen like that. Always resulting in someone dying.

Sweden lowered their already low death rate by widely installing guard rails in the middle of rural roads to separate the lines. Probably one of the smartest investions the state could realize, at least on high risk rural roads.
