This was revealed at Super Hero Hype yesterday. Read at your own risk:

Batman/Gordon/Harvey Dent and Rachel Dawes set up a trap for the Joker using Rachel as bait. It totally backfires and Joker discovers Bruce = Batman and murders Rachel.

so wait why is the girl coming back playing Katie's character?  Couldn't they write her out of the script and have this new girl as another girl?

I just hate when they replace actors for the same roles.

Because Rachel has a connection to Batman and the character needs to work for Harvey Dent.

so was vickie vale ...well not really, but sorta.  She knew who Batman was in the film, and bam she's just gone in the second one...

I pretty much knew this once they re-cast her character.  Nolan would only change actors if he really, really wanted that character and the only way he'd want her back that bad is to kill her and connect Dent and Joker.
