I think for the rom com to be truly successful
I have to be PRETENDING to be sick to meet med school girls
And maybe have a bet with one of your buddies about how many chicks you can date/diseases you can fake.
Then she finds out at the beginning of the third act, and is all "I can't believe you lied to me!" and you're all "It was a game at first, but I fell in love with you and just couldn't find a way to tell you!"
You prove your love through a grand gesture of selflessness, which saves the community hospital from the evil developer, or the sick kids' ward from the evil chief of medicine/romantic rival. She sees you on TV repeating a poignant line that she said to you earlier in the movie, and realizes you
do care.
