1. I dont know if either has actually spoiled anything. I havent read the book and the trailer still makes it look blatantly obvious that Leo is 67.
2. A movie isnt good in the first place if knowing the twist ruins it.

Mental asylums are a great place to scare the shit out of you, especially by the look of this one.

I don't think Leo being No. 67 is the main twist of the film. That would be too hokey, something reserved for a contrived slasher filck - not a Scorsese picture.

Could be the plot revolves around no. 67, and is a character study into Leo's descent into insanity?

If Scorsese's name wasn't attached, I doubt this film would be getting much hype.  The trailer makes it look like a pretty by-the-numbers thriller.
