This was revealed at Super Hero Hype yesterday. Read at your own risk:

Batman/Gordon/Harvey Dent and Rachel Dawes set up a trap for the Joker using Rachel as bait. It totally backfires and Joker discovers Bruce = Batman and murders Rachel.

I pretty much knew this once they re-cast her character. Nolan would only change actors if he really, really wanted that character and the only way he'd want her back that bad is to kill her and connect Dent and Joker.
