What are the ramifications if they test positive for a disease (I doubt many prostitutes would voluntarily quit their profession when told they've contracted herpes)?

This is why I've never felt the urge to get a one. For the same amount of money you would pay, you can usually get some the old fashioned way. There is also no chase, but I guess that is preferable to some.
