Pretty sure I've been dealing with a drug abuser at the office. Last night a patient called saying her friend (also a patient) was in severe pain due to a dental partial, and needed an emergency. I called the doctor, he was out of town but authorized me to call in a script; this is where shit gets suspect. I asked her if her friend was ok with Vicodin and she was like "it makes her nauseous, and we're both recovering addicts so I don't think it's a good idea to prescribe that" (red flag 1). So I offered Percocet, which is slightly less potent than Vicodin and doesn't have hydrocodone in it (which causes nausea in some people). She then proceeds to list off some narcotics she'd rather have (red flag 2), none of which I'd be able to authorize.
After some haggling I got her to accept Tylenol 3, 16 tablets; plus set up an appointment for Tuesday. That was last night around 7pm or so. I had to go into the office today because a repair guy had an appointment to upgrade our x-ray machine. So around 11AM the patient calls back saying her friend is still in pain and is worried about running out of pills. She says she has taken 4 so far, meaning she has 12 left.
me: so she has 12 left?
her: yes but she's gonna run out on monday. can you refill the prescription
me: I cannot refill a narcotic prescription less than 24 hours after it was filed.
her: WHY NOT
me: because that sets off red flags with pharmacists, and the doctor is concerned..
her: he's more concerned about a PHARMACIST than his own patient??!?! are you serious!?!?
me: yes, it's a serious thing...doctors can lose their licenses -
her: his license? my friend is in pain?!?!?! she needs meds
me: but she has 12 left...
5 more minutes of that plus her saying they're reconsidering coming back ever again smh. If you're in severe pain, why can't you take a day off from work to get the earliest possible appointment on Monday? The doctor can give you a sick leave, jesus. Instead she's coming in LATE on Tuesday. It doesn't make any sense, and given the fact that the person in pain never spoke on the phone and how rabid her friend sounded...I'm almost certain they just wanted drugs.
